Long Term Conditions

We encourage patients who suffer from the following long term conditions to attend for an annual  review in the month of your birthday:

  • Asthma
  • Chronic Obstructive Pulmonary Disease
  • Coronary heart disease
  • Diabetes
  • Epilepsy
  • Hypertension
  • Hypothyroidism
  • Kidney disease
  • Mental health (including depression)
  • Stroke

Diabetic patients are invited to a "one-stop" clinic to see a practice nurse, dietician and doctor all in one visit.

Health Promotion/ Disease Prevention

We are actively engaged in health education and disease prevention and offer the following:

  • New patient health checks
  • NHS heart health checks (age 40-74)
  • Health checks for the over 75s
  • Smoking cessation advice
  • Travel advice and immunisation (including Yellow Fever)
  • Vaccinations and immunisations (including childhood and Flu)
  • Contraception advice
  • Menopause management
  • Child health & development checks
  • Weight monitoring and advice
  • Sexual health advice including chlamydia screening and C-card scheme (under 25s)
