Crocus Medical Practice participates in My Care Record, a secure system that enables authorised health and care professionals involved in your treatment to access relevant information from your health and social care records.
The aim of My Care Record is to improve patient care by ensuring that the professionals looking after you have access to accurate, up-to-date information when and where it is needed.
Traditionally, information has been shared between healthcare organisations using methods such as post, fax, or secure email. These processes can be slow and may delay diagnosis, treatment, or care decisions.
My Care Record provides a faster and more secure way of sharing essential information across participating health and social care organisations.
How My Care Record Works
My Care Record does not create a new medical record. Instead, it allows authorised healthcare professionals to securely view information held within existing NHS and social care systems.
Information is accessed directly from the organisation that holds your record and is only available to health and care professionals involved in your care.
For example, if you attend hospital, clinicians may be able to view relevant information from your GP record to help them make informed decisions about your treatment.

- NHS 111 and Out of Hours providers
- Approved social care professionals
All access is strictly controlled, monitored, and subject to NHS confidentiality and information governance standards.

My Care Record:
